The questionnaire is also online, you will be given the link once you have sent the email for the lozenges.
Prize details:
This prize is a pamper break for two people, including an overnight stay, at a choice of Marriott Hotels in a twin or double room. Prize includes breakfast, full use of the spa facilities and two spa treatments per person from a choice of: Back, Neck & Shoulder Massage; Facial or Mini Manicure. Prize is subject to availability, and must be taken before the end June 2010, excluding bank holidays, Christmas, New Year and Easter. The prize is non-transferable and there is no cash alternative. Winner must be 18 or over.
