The funding request and the clinical evidence to support it must come from the treating clinician - has your dentist indicated there is nothing else to be done / s/he's tried every treatment option?
I have heard of very few people getting approval for this, and it is usually young adults who have lost front teeth as a result of accident, or genetically missing teeth, or problems like cleft palates.

1. Get recommendations from friends/family/neighbours/etc.
2. Once you have a short-list, VISIT the practices - dont just phone. Go on the pretext of getting a Practice Leaflet.
3. Assess the helpfulness of the staff and the level of the facilities.
4. Only book initial appointment when you find a place you are happy with.0
