SOA for Bankruptcy on Tuesday

My husband and I are going bankrupt on Tuesday and I would really appreciate it if you brilliant people could look over our soa and see what you think!
A little confused in that the expenditure form on the bankruptcy form doesn't let you include things like hairdressers and school dinners etc. Do we just add these to the bottom of the sheet?

Partners monthly income after tax....... 1250
Benefits................................ 186 Family Allowance 41 child tax credit but this is due to drop to £20 in a couple of months.
Other income............................ 0
Total monthly income.................... 3793

Monthly Expense Details
Mortgage................................ 430
Secured/HP loan repayments.............. 916
Rent....................................
Management charge (leasehold property).. 0
Council tax............................. 104
Electricity............................. 130 including gas
Gas..................................... 0
Oil..................................... 0
Water rates............................. 33
Telephone (land line)................... 50 including broadband
Mobile phone............................ 30
TV Licence.............................. 12

Groceries etc. ......................... 800 We have a baby and are still buying nappies and milk

    Hope that helps, you are allowed £5 per child per week for a activity, £60 a month, what ever school dinners are. After school clubs can only be claimed if they are used for childcare.
