More filling form questions, please help!!!!!!!

I am having a nightmare with these forms, i am so sorry to keep asking you guys questions but i just cant seem to find the info i need.
My problem is the forms include an 'employment and present income' form
it only asks about paid work, there is no where to put what benefits we recieve, where it asks for 'take home pay' (including overtime etc) should i just put our jsa amount in that box?
Also we was told by our advisor we could get help towards costs, she didnt include the form we need to fill out in order to get the help, this is the form we need...Application for a fee remission - EX160
Can i get one to print out online anywhere?
And do we have to apply for this BEFORE the day of banckrupcy or just take it on the day with the rest of our forms?
Again i apologize for so many questions!

are you doing your forms by hand or online i found it better to do it in word from downloading the forms online if you need a program download open office its free and opens word programs this is how i did mine.Thankyou Oregonuk1 i just found it, thats one less thing to fret over:o0
