I've never read that I do but just wanted to check. Also, can the forms be printed double sided?

    No ID is needed. Double sided printing will be fine. This was how the court I used sent them to me.

    Hopefully....most dont usually but for some reason Liverpool asked for the info thankfully I dont live too far from the court otherwise i'd have had to reschedule the appt...:D
