    As The Pedant (wish I had gone for than user name) points out an "accident book" is not required but a means of recording any accidents/incidents is.

    The employer has a duty to investigate the accident and there should be recommendations and outcomes from this investigation.

    There should also be a (possibly generic) risk assessment for the work being carried out, which is designed to highlight potential problems and show the control measures which have been put in place.

    As others have already said this is worth taking further

    well - update is that DD still has no word form her employers but has spoken to a solicitor, who is sending her an information pack.
      To those who are given much, much is expected
