problems with form!

hiya. Im really hoping someone can help me with a few questions on the bankruptcy form. i went to the CAB and the man i spoke to seemed to have less of an idea than me, which i was suprised at because they have helped me with various stuff before
anyways questions.
1. I am applying for the fee remision, and in the leaflet it says i need "3 months bank statements from all current account showing credit of all income" does this mean i need 3 months bank statements from all banks or only the bank i have money paid into?

2. My boyfriend and i are self employed. We only stated up the business in june, and stupid me hasnt really been keeping accounts. do i need to sit and write up all this info to take, or do i not need to?

3. on the list of secured creditors i had started filling it in with unsecured ones, i have no secured creditors. i have crossed out secured and written unsecured in its place. will this be accepted?

4. I cant remember when i got alot of my loans and catalogues, it asks for date obtained. can i leave this blank?

5. on list of unsecured creditors it asks for account numbers and addresses of them. i can get most account numbers but not all, and not all addresses, is it essential?

6. in causes of bankruptcy.when did i first have dificulty paying debts.i have always had dificulty, since i fist got a catalogue at 16, so do i just write 16?

7.reasons for not having enough money to pay debts. from what i can gather i be very honest. tbh i originally got a catalogue at 16, no intension of paying it, was young and very stupid and wanted new clothes! im thinking maybe thats a bit to honest? if so how far back do i go for having reasons for not paying debts. the honest truth is basicly i was unemployed firstly at 16, wanted some nice things. at 18 had my son, had very bad postnatal depression, so got loans and catalogues and didnt think of how i was going to pay them. i have bascily been tryng to avoid and run away from then all my life, but now im older, and have settled where i am, they have found me! all the debt totals about £22000, and i can only see bankruptcy as way out.i have spoken to nat debt lne, who are lovely people, but closed at 1pm today, and im worrying myself so thats why im asking on here.
I was also told by the cab that the 3 reasons not to use are depression, unemplyment and being young and stupid!.

8. the lady who gave me the foms at court, said all letters have to be dated within 1 month or going to court. but not all the letters i have are, for example child benefit entitlement letter was dated when my daughter was first claimed for almost 21 months ago, will this be accepted, and the housing benefit and council tax benefit letters were dated from april this year how about them?

9. what happeneds to any housing benefit or child benefit paid into my bank(that will be frozen i presume) after the hearring(which is thursday)? i have had child and working tax credits paid to my boyfriends account, but child benefit takes ages for forms to change details to come and to get back and be processed. i cannot affords to loose the money, or not have access while the change of bank account gets processed.

sorry for so many questions! im just sat worring myself silly over this

    Sorry I can't help with most of your questions but you can download a copy of the forms here if you want to print them off and start again.

    To be on the safe side I would send copies of all accounts you have had monies paid in to if you are trying to claim the fee remission.

    As for the date when you first had difficulty paying the debts although you may have always had difficulty I would be tempted to put the first date you couldn't actually make payments. Otherwise you may have to explain why you have continued to use credit knowing that you may be insolvent.

    As you have said be as honest as possible with the reasons for BR (I would leave out the "no intention of paying it" part). The OR will end up finding out what happened anyway so don't risk a BRU/BRO by not being completely honest. The OR will have heard it all before anyway so don't worry.

    I would try and get as much information together beforehand and get the forms filled in as complete as possible so that the OR has a better picture before the interview as it will be easier on you then. You won't need the detailed account info for your business until the OR interview.

    Any information for your loans ie. dates the OR can get from the creditor themselves, just try and get the names/addresses and ref no.s on the forms for them.

    I only went BR at the beginning of this month and I was so worried about the forms/BR hearing/ OR interview (still got OR interview to go) but as everyone says the actual experience is no way near as bad as you imagine.

Just the ones you have benefits paid into.

You wont need it for court, but you will need to show what your earnings have been for the OR interview that happens a couple of weks later.

I have seen that dne before and it wasnt a problem, you can download fresh copies though if you want to make it neater.

It is best not the OR is tasked with investigating how the credit was incurred and deciding how long and what type of interview you have, the more infor you give in the forms the easier it will be at the interview. Try and link your debts to events in your life i.e was it takn out to buy something specific, holiday, christmas, marriage, birthday etc. Try and be as acurate as you can, for loans put down the date the loan was taken, for creditcards/catalogues put down when the majority of the money was spent. e.g might put something like "in the year prior to jan 09".

As above it will help the OR and make your bankruptcy pass quicker and easier. t also ensures that all the credtors get the correct report and so stops you getting hassled latter down the line.

You will be allowed to keep the money that goes in after the hearing, sometimes accounts d get frozen, but this is an administrative process not the OR saying you cant have the money, it is best to talk to the OR as soon as possible about he bank account. ven if the bank shuts the account they should let you have that money.

    On 4 &5, info on start dates and addresses (not ref nos) will be on your credit file. I realise you don't have time to get a paper copy but you can maybe sign up (and then cancel) for an on-line version

    On 6&7 I would put the date when you first realised you needed help and contacted National Debtline/CCCs/CAB about your debts. I also wouldn't write an essay on the reasons - just the basic points in one paragraph that fits in the box provided.

    On 8, if you are going for remission on annual income you need to follow to the letter the requirements of your EX160A

    The same site will allow you to download a replacement page for your unsecured creditor list if you want

    Just noticed the EX160A says:

    Bank Statements
    Applicants applying for remissions 2 and 3 must provide the last 3 months bank statements from all their current accounts showing credit of all income and all other regular credits and debits. Failure to provide bank statements will result in your application being refused.

    I'd take statements for all accounts to be safe.

    wow thank you so much for answering my questions, you have all really put my mind at ease!
    1 more quick thing, on the day i go to court(thursday!!) forgetting about the fee remission form stuff, what do i actually need to take with me? is it just the forms ive filled in or do i need id,bankstatements, proofs of income ect?

    You just take the forms and your money.

    Also take a book/mag as sometimes you have to hang around. Also a good idea to have some change for the vending machine or a drink and a snack in your bag.

    You will be fine. If you haven't got the fee remission stuff together by the time you go BR then you pay full fee and then can claim back the £150 later.
