is bread machine on the way out

twice now my bread has failed to rise, ive followed recipie extactly as i have done many times before now. i usually get a lovely well risen nice tasteing loaf
it comes out resembling a house brick on the outside the inside is stodgy and very dense (i think thats best way to describe it)
i am using hovis fast action yeast (in the sachets) could it be a dodgy batch is there any way of testing it before i waste anymore time adding it to my bread?
please help before my machine finds itself out for the binmen tomorrow

Is your yeast fresh?
When I have had problems I have bought fresh yeast and made a loaf with that and it always turns out to be the reason, even if I think the yeast I already have is fresh enough.
If you are would replace your bread machine if this one has given up then I would suggest your first step is to buy some fresh yeast and try that. If that works and your current batch of yeast is not very old, I would be inclined to take it back to where you bought it and ask for a refund.0
