Help with SOA 6.28

Just a little background info first.
I'm a self-employed private hire taxi driver. Which means I work for myself but I work from a company who provides me with the work from phone bookings, in return I pay them £95/week irrespective of how much I earn/do not earn.
Now for my questions regarding completing the form.
Name of Business - do I just put my own name here as I don't really see myself as a business? Address of business I guess will be my home address.
Assets - I'm guessing this is just my car (which I own). I have no building assets etc.
Unsecured Creditors - The date debts incurred, does this need to be exact as they have been building up over the years?
Present Income - Take Home Pay - I assume this will be after all of my costs, eg. office rent (mentioned above), fuel, repairs, insurance etc.
I spoke with the people at the CCCS in August and they advised Bankruptcy as my option but I've tried to avoid it, but I can't any longer.

Yes, if you do not have a trading name, then this is your name. You should not put down the name of the company that supplies the work unless you are an owner/director of that company (and then you would need more specific advice)
Assets - I'm guessing this is just my car (which I own). I have no building assets etc. If you do not own your own home, and have no other assets worth a significant amount then that is correct. If you are a homeowner, then you will need to include your home as an asset, and the value of your share of any equity will form part of your bankrupt estate.
Unsecured Creditors - The date debts incurred, does this need to be exact as they have been building up over the years? I put down the dates that each agreement started, regardless of the fact that they built up. Most credit card and loan debts will show the start date on your Experian or Equifax credit report.
Present Income - Take Home Pay - I assume this will be after all of my costs, eg. office rent (mentioned above), fuel, repairs, insurance etc. This should be your net profit, so yes, after everything has been subtracted. The deductions from gross turnover should be the same as for your self assessment tax return.

Hi, I would imagine it would be under the specific motor vehicles section, and maybe cross reference this in tools of your trade. If you put it in the wrong section, the examiner will discuss it with you during the interview and will make any changes that are necessary.

Tools of your trade are usually exempt as they are necessary for the sustainability of your income maybe this will include the car. Heres a link that shows a completed 6.28 form.

Sorry to hear that you have not been able to work around the BR situation. Well done on trying but sometimes you just know when it is the last resort & unavoidable.

1 more question, I've got a Barclays cashcard account and I don't owe them anything, will this be frozen as soon as the judge signs the forms or will I still be able to use it for standing orders etc?0 -
You should be OK
I don't have any experience of the Barclays cash card, but I did open my Co-op cash minder a month before I went BR, and it was allowed to continue. BSC 2710 -
Just doing my list of creditors and I've got a CCJ from about 1997. The amount was £10/month which if I remember correctly would've took me over 40 years to repay.
The problem I have now is that I have no address or contact details for who I'm paying it to. The only name I can remember was the solicitors acting on behalf of Barclaycard and that was Elizabeth Handforth.
Does anyone have any contact details for her? I've tried google but it isn't coming up with anything.
