DD under 16 - first job - help

My 15 yr old DD has just been offered a job for a couple of hours a week.
Her new employer has said I need to find and complete some tax forms to make it all legal & above board.
What forms do I need and where can I get them?
Is there anything else I need to do or be aware of?

When I saw a kid many many moons ago now, I had to complete a permit from the county council which is mentioned in the above site.0 -
I would have said that the employer needed to give your DD a P46, which can be downloaded from the HMRC website, but I do always say that if the employer looks blank at the mention of a P46 and you don't have a P45 for any reason, you can download it yourself.Signature removed for peace of mind0

The Inland Revenue require a P38S - student employees.
You can download this from the Inland Revenue web site - I have just looked at it myself.
You will need to ask the employer for their PAYE reference.
In an ideal world the employer should be providing the form.0 -
saver_sam is correct. Under 16s require permission for local council and/or school in order to work. This is so that is does not interefere with studies.
I doubt the above 2 forms would be useful as you do not get a NI number until you are 16 (or 3 months before).0 -
Unless they've changed it, the P38S only applies if you're ONLY working during holidays. If you're working during term time too, it's not technically valid.The Inland Revenue require a P38S - student employees.

To be fair, he runs a small business and is going to consult his accountant about how do make sure all is legal and above board - but I just wanted to know if I needed to do/sing anything.
