Not sure how it needs to be set out on the postcard. There is a form in the mag and you tick the boxes of the comps you want to enter. On the form they need name, address, postcode, age, email address, telephone and signature of parent/guardian and T-SHIRT SIZE (SMALL MEDIUM OR LARGE)
