cannot download remortgaging pdf

hello,
after filling in the form on http://www.brochurecentre.co.uk/ms3.asp and submitting it, I just get:

Microsoft OLE DB Provider for ODBC Drivers error '80004005'
Can't open file: 'fb_information.MYD'. (errno: 145)
/checkresultsMS3.asp, line 54
----
sorm form of db problem that I cannot seem to circumnavigate, so I cannot get the pdf. Any chance of getting this fixed?
