Deed of assignment for endownment

Hi all,

I have a few questions reference "Deeds of Assignment" for endownment policies

Can anyone provide a link where I can get a free template that is legally binding.

Once this is completed by both parties can it be witnessed by anyone or does it have to be a solicitor?

Can both parties sign separatley (not speaking due to divorce)

I am trying to do this without encurring any legal cost. I have split up with my partner and they are signing over an endownment to me.

Have I missed anything?

    They say they don't do them and I should go via a solicitor. The solicitor route is over two hundred pounds, which I am trying to avoid.

    What life company is it? Most have default deed of assignment forms. Indeed, I got one from Clerical Medical a few weeks back.

    It's an endownment from the CO-OP.
