Deed of assignment for endownment

Hi all,
I have a few questions reference "Deeds of Assignment" for endownment policies
Can anyone provide a link where I can get a free template that is legally binding.
Once this is completed by both parties can it be witnessed by anyone or does it have to be a solicitor?
Can both parties sign separatley (not speaking due to divorce)
I am trying to do this without encurring any legal cost. I have split up with my partner and they are signing over an endownment to me.
Have I missed anything?
Thanks in advance for any advise.

What life company is it? Most have default deed of assignment forms. Indeed, I got one from Clerical Medical a few weeks back.I am an Independent Financial Adviser (IFA). If you feel an area discussed may be relevant to you, then please seek advice from an Independent Financial Adviser local to you.0
