CSA form dpu15F

Hi

I requested my DP from the CSA and recieved form DPU15F this asks me which departments I have dealt with ( I have so little correspondance I am not sure which deprtments I have dealt with ) what am I best of putting to ensure I get all info.
It asks if I require only specific information to which I presume the answer is all information.
Third point is asking if I want date specific Info again I think it should be all information.
And the last bit is about email to or from them to which there has been none.

What is the best wording to fill in this form to ensure I get everything including any post it notes etc.

    The only possible answer that I can think of to your first question is "all". How can you possibly know what departments you have spoken to unless you actually work there?

    Hi, My husband was sent DPU15F last week. We couldn't remember which departments we had delt with and they only gave one example on the form ( enforcement) so we looked in one of their leaflets and just copied all of the departments mentioned in leaflet.

    We wrote enforcement, collections, criminal compliance and customer services ( that wasn't the correct word but can't remember it!)

    I know someone who had the same form, and just wrote ALL departments, and only received the last years worth of payment breakdown as they hadn't been detailed enough! :rolleyes:

    This must be something new, maybe all the CSA staff are actually doing these days is printing out DPA requests. I never got this form I did write a letter requesting all data and got years worth :D

    :think: cunning ploy maybe to try and confuse the peson requesting the data.

    We got this form, as I know others on this forum have, it seems to be considered as a delay tactic on behalf of CSA. Surely if you state in writing, recorded delivery that you want ALL INFORMATION from ALL DEPARTMENTS, this is precisely what you mean!
