Ex-partner, loan and IVA

A friend of mine is desperate for some advice on a situation that has arisen.
Her and her ex-partner (split approx 15 months ago) did have a Halifax mortgage and a £11k Halifax joint loan together. About 5 months ago he managed to refinance the mortgage so only remained the joint loan. This loan was arranged prior to them breaking up and was to facilitate paying off some of his debts and it was agreed that he would pay it until completion
It now transpires that after he got the house to himself he entered into an IVA and has had his debts satisfied and the joint loan has been transferred into solely my friends name.
My friend changed her address with Halifax but there remains an old address on the loan so she was not made aware of any of this until a CreditExpert alert popped up a couple of days ago.
She contacted the Halifax to enquire, only to find out it has now been passed onto a DCA. She has no means to pay this at all and has only her own debts.
My friend finds it hard to believe that it could just be passed into her name without her knowledge and is now worried about the DCA coming after her.
What would be the best way to sort this out? Has the IVA been administered correctly? Are Halifax in the wrong for having the wrong address for the loan, as they had the correct address for the other Halifax accounts my friend holds? Now the DCA are involved would they accept a lower sum to clear the debt?
