  • Is lifting part of your job? Was the injury recorded officially? Have you discussed this with your union?

What were you doing when you injured your back?
Was what you were doing part of your job?
    Have you seen your GP?

And as above everything that Oldernotwiser says
  • Hi

    to add to juliescot's questions

is lifting and handling training not part of the induction process for all new employees?

how long have you worked there?

is there a risk assessment for the activity you were doing?

What were you doing when you hurt your back?

is the activity part of your normal duties?

are you a permanent employee or Agency?

is it recorded in the accident book?

  • Enter it into accident book and add the words 'lack of training'.

To whom did you report the accident? You don't need to fill in the form/book yourself.
Do you have a safety officer/advisor someone who would investigate the accident?
What did the people say A&E say?
    What does your GP say?

Was what you were doing part of your duties?

If you are off work, for more than 3 days, as a result of this accident the company have a duty to report it http://www.hse.gov.uk/riddor/
