I'd imagine the barcode is the same as all (for the sake of them scanning) - I have edited my name out of it, so you might want to edit yours into it.

Here it is in better quality (GIF), and with a sample name put in as mine was originally. I recommend Arial font, size 11... aligned to the right like this sample. It says you can print as many as you need, just you will need ID.
