    Looking at copies of references that my boss has completed recently, most of them have a simple check list performance/honesty and integrity/punctuality/health and attendance record where you tick excellent, good, average or poor. Most ask for reason for leaving and employment dates and 'would you re-employ' is a common one too.

    I have one which was far more detailed with all sorts of stuff on it.

    So far as absence goes they may ask how many days sick leave in past year or have a check box as above.
