Do I have to declare the £100 bonus I receive from the bank because I opened an account with them (A&L or First direct i.e.) on my self assessment form as income?

No. This is treated as a cash discount and is not a taxable amount.
If it were, the bank would give you a certficate of tax deducted but I have not come across any case where this has been applied.If it’s not important to you, don’t consume it0
