LX Direct help!

OK. I used a £15 code a few weeks ago for a shirt. The shirt was £16.99. So they charged me £16.99 and £3.50 in 2 transactions (weird.)
Then they refunded £15.

However the shirt was too small. I sent it back and ordered a new one at the same time. The lady said they would have to refund the first shirt and charge me again for the new one (even weirder.)

I mentioned about the £15 and felt a bit fraudulant when she said "Oh yes you used a £15 VOUCHER."

They refunded the £16.99 and left the £15 deduction on. BUT. Didn't refund the postage. So I have been charged twice? Is this fair? I always thought returns were completely free??
