    sometimes as well a pre-pay envelope to send any requested bank statements etc (dont worry if you've no statements credit agreements etc the receiver can source them easily)

    NBT2 - Notice setting out your duties and responsibilities as an un-discharged bankrupt.

    A copy of that has to be signed and returned.
