Have put in my full name and address and occupation, my names I have also been known as,.......do I fill out my address again in part [c] lately residing at?
part [d] and [e]....dont apply as I have not been carrying on a business.

1f...my centre of main interests has been...I have had an establishment at?......what do I put here?

in h...I guess I tick I am unable to pay me debts?

I did enter into an IVA...do I state that along with date in [ j]?

I ummed and arred over this question too so would be interested to know what others say....My query was the 'all names you have known by' bit. This, because I have a double barrelled forename (oh yah) and sometimes I use my complete name and sometimes just half of it. because the names are both mine I can legitimately use either/or and both.

And do you have to state your maiden name too?

Anyway, now I have hijacked your thread (sorry) and done everything but answer your query, I hope someone comes along to answer....you first of course, I don't wish to be rude!

I've stated my maiden name and my previous married name......I would imagne that you'd state your forename in full or maybe for example.....

Yes, thats seems logical and with any other form I wouldn't think twice about setting it out like that.

There's just something about these BR that make me feel nervous about getting anything wrong! I'm pertified I will confuse the OR in any way and they will punish me somehow!

'lately residing at' means previous adress (if you incurred debts at that address)

In my local court the bankruptcy clerk prefers to complete page 2 as she has to ask those questions anyway and doesn't trust me to delete the right bits!
