Applying for BR. What forms and where?

Can anyone tell me where I can get the forms sent to me? I do not have a printer.
Also, I am applying for personal BR as well as a business. The business is a partnership (not limited), and the bank debts are unsecured in both names (we are both applying for BR). Can anyone help me with the type of forms required to apply for BR.
