Form 6.28 - advice please

This is my second attempt at asking for help/advice with questions on the Form 6.28!! The damm computer didn't list it in the Forum......so I'm asking all over again lol

Personal Details - I have written mobile no. Should I cross it out as I don't think I should be even using the phone?? I am currently on contract with it, but runs until Jan, and wondered it this should be added to the list of creditors??

Section 3

3.1 Cash in hand - I don't have any spare money if that is what it means??

3.2 Cash in bank etc - I've closed my bank & building soceity a/c's and the money in there is being used to pay the BR court fee. I've opened a new basic barclay a/c, so should I just enter what cash is currently in there?

Assets
3.2 Endowment Policy - had one 11 years ago when was married. But cashed it in with ex when got divorced, do I still need to list it?

Section 5 Bank a/c
5.1 - do I list the one's I've just closed or the barclays one I have now?

Section 6 - Emloyment

I am in receipt of income support, and have put last employers details in.

6.9 last income tax ref no. what is it, and where can I find it?

Section 7 - Outgoings
Shall I just go by the SOA you guys on here amended with me a few days ago?

Section 11 - causes of BR

when first had difficulty???? I've being paying up until recently, CCCS advised that it would take me 30+years to clear debts and recommended BR.

Reasons for not having enough money??????

Your help/advice would be gratefully received x
