i've been asked to provide bank statements etc.
they sent me a form which requires a number for each item book/record
do you have to identify each page of the bank/credit card statement with a
number? or just the whole load of statements together as one number?

I would lump them together - so BArclaycard Statements clipped together and marked on the front ITEM ONE - the Natwest bank account clipped together and marked ITEM TWO; etc. I see each institution as one record; one account is one record really.

The schedule (form) has 'date of first entry' & 'date of last entry' at the top Longshot - I numbered everything I sent them on the form as in 1) **** Bank statements 2) Home insurance Policy, 3) Payslips etc., with corresponding number on the front of the actual paperwork then for statements/payslips date of 1st entry (1st statement/page) to date of last entry (final statement/page). I didn't have c/c statements for the whole of the past year so I just sent them what I had (which was fine) - keep a copy of the form for yourself & remember to send by registered post
