Form 6.27 - B/R Petition

I’m sure it’s just me – but I’m finding “The Debtors Bankruptcy Petition” a bit difficult to fill out.
It’s Form 6.27.
On the second page -
Section 3 - I am unable to pay my debts - do you just put a tick next to the points that apply?
And then cross out all the stuff below that does not apply.

No I've got the papers in front of me.
It's the single sheet - Debtors Bankruptcy Petition - Form 6.27.
It's printed on both sides.
The first side has all your name and address details.
It's the second side that's confusing me.

If i remember rightly the court usher filled mine in for me. Yes it is.
But even if you haven't changed your name - you still have to fill the form out.
It came with the SOA form in the Bankruptcy Pack from the local court.0 -
Yes i printed my copies(6.28) off the IS website and never knew about that one:o,thats why the usher added mine. If you are not sure leave it till you get there. Unless anybody else on here knows:D.

(g) fill in the town of your court
(h) state reasons ........she put the court...moon town:D
3.she crossed out "i was adjudged bc on"
she also crossed out "ON(J)............I(F) made a composition ect"
she left i have not entered into a voluntary arrangement
crossed out "on(j).........i entered into
left (iv)i have not been subject to
crossed out on (j) .......an administration order
5. a statement of affairs
date and leave signing until you get there
