Site with financial problem template letters

I've been to a site at some point that had some great "I'm in financial difficulties, please stop charging me interest and charges while I sort out what I'm going to do" templates.

Can anyone point me in the right direction?

    Dear Sir/Madam

    Re: − Account/Reference Number: 0000000000000

I am writing to inform you that I am seeking advice and assistance regarding our current financial difficulties.
I am writing to all my creditors requesting details of the balance outstanding on my accounts and would be grateful if you would let me know how much I owe
To this end, I would appreciate if you could hold any action on the above account, and also could you please freeze interest and/or any other charges accruing on the above account.
I look forward to hearing from you as soon as possible.

    Yours faithfully
