I want to be debt free - Closing Credit Card Accts - Standard Letters?

Dear All
I am new here but not new to the world of debt.
I am in the process of compiling by Statement of Accounts so that I can offer payments to credit card companies.
I have searched a few times but I can not find a link to standard letters that help me to say... close my account, freeze all interest charges and will you accept xyz per month, heres what I am paying everyone.
Can you help? I have tried using imoneymanager but I appear to have a lot more disposable income that my bank balance doesnot know about.
I have my bank charges claims in, they have been in for about 18 months. I am part way through claiming credit card charges and loan charges.
I am back using Microsoft Money to track stuff and I have started a daily spending book which is helpful.
I would be very please to receive any letter pointers.

I have been in touch with my banks and building societies and replies are coming in.
The Royal Bank of Scotland (I owe approx £1200) have told be that as my payment offered is more than the 2.25% minimum payment, they are unable to freeze my charges/interest.
