thanx for this, but i must be really dumb, coz the boxes dont tell me what to write in them, and ive tried my name and address, but the bottom 2 boxes are empty and it wont proceed without putting something in them, do you know what info they want?

The two bottom boxes are 'email' and 'confirm email'. It looks like this (copied and pasted):
