CCJ Redetermination help

I have had the CCJ result through the post this morning, as I'm after delaying tactics (and I can't afford the monthly repayment they've set anyway) I'm going to reply and ask for a redetermination that was mentioned to me before in another post.
My question is- do I need a form to accompany the letter to the court asking for a redetermination or can I just send the template letter that was suggested to me?

No, you can just send the letter, also there is a time limit (14 days springs to mind). The payment level will then be determind by a district judge, with or without a hearing.

many thanks, the judgment sheet mentioned I have 16 days from the date on the envelope, presumably this is to take into to account for 2 days in the post.
I'll get the letter and budget sheet sent out today recorded delivery.
