New to Tesco Clubcard Deals? Free 500 Points

My last thread was deleted for no apparent reason. If the moderators have moved the thread please let me know were to ? if deleted please let me know why ? :mad:
FORM HAS BEEN SENT OUT, NO MORE AVAILABLE, other members feel free to use this thread to giveaway any spare forms you have.
I have ONE form from Tesco's which gives 500 bonus points if you are new to Clubcard Deals, i.e. never converted Clubcard points to Clubcard Deals tokens.
The form has to be used by 14th April 2006.
Please private message me if you can use the form by 14th April 2006, i.e. have paper clubcard vouchers you want to convert and know the Clubcard deal you are ordering. If you supply me with your name & address I will send the form to the first person.
I have received and sent private messages, but as yet no one has supplied their address for me to send the form to, first come first serve.
