i also got foundation today and light purple eyeshadow yesterday. the eyeshadow is wonderful but the foundation is WAY too dark, looks like i have a bad fake tan. i do have super pale skin though.0

you put your name on the questionnaire because its your name that they have on file.

Me too, I always write my name and number on the front cover, and then just get him to fill out the rest of the form.0
