I done a questioneire thing and it said would need medical form to be filled in and one would be posted out, not had it yet though but post is up the wall so fingers crossed may still come.

from what I can remember, the medical forms were needed for the people available to attend trials at the Product Evaluation Suite in Nottingham.

lexuslass is right, the medical forms are only for people attending the trials in Nottingham.

I got that this morning, strangely it had two labels on it, when I took the top one off it said 'Surf Spray' but with the same product code...
