If you hover the mouse over the question mark next to the box, it says - "Please make sure there is a tick here to recieve your free gift"

Just ordered mine, wasn't asked for payment.
"sent by return" means sent straight away - i.e. if you're replying the same day to a letter you've received it's sent by return (post).
