Lump sum of pension not being paid

Hello all, I am a newbie here.
I was wondering if any one could please help my dad out.?
There is not a lot of info to go on but here goes....
My dad retired a couple of years ago and did not here anything from his pension
untill I phoned them for him.They sent him a few forms to fill in and he returned them.They then sent him a form asking him did he want to cancel?
We did not do anything with this form and did not here anything for a while.
Anyway to cut a long story short, They are now saying that he will NOT get a lump sum because he did not cancel within 30 days.It looks like he has lost out on around £7000. Can someone please help? I have been trying to phone on behalf of my dad (he is hard of hearing and gets frustrated) but they will not talk to me.

What type of pension scheme was it?
If personal scheme, what type of annuity was your father arranging and on what terms?
Did he utilise the open market option or arrange the income through the same company?I am an Independent Financial Adviser (IFA).
