I do hope you can change his mind. Reassure him that it's totally anonymous. Sometimes it's just good to get another perspective on your situation, other people can spot opportunities which you might miss yourself because you are so stressed.
Even so, admitting the problem is the hardest part for most of us. Once you are resolved to sort it out everything else slots into place. Good luck
