Tax Return Query

Hello,
Am soon going to fill in my first online tax return. I worked under the CIS and 20% tax was paid by my employer.
Couple of questions.
Is there somewhere on the form where I explain that I was a CIS worker and that this amount of tax was deducted or do they somehow know?
I am still waiting for a final payment from last November. Not looking likely that I will get it. It is for a few thousand pounds. Do I mention this.
Because of this situation and because I am not personally in contact with the person who owes me this money. Does he have to send me some sort of record showing that my 20% tax was paid?
Any help would be appreciated.

There is a space on the self assessment where you enter CIS tax deducted. If you have not received a payment, then I assume you have no evidence that tax was paid on it. You cannot enter it on the form.£705,000 raised by client groups in the past 18 mths :beer:0
