Worried about CCA request from MBNA

MBNA have sent me the paper work but im still unsure whether its legal (sorry i cant post this & i have no camera ) It says i applied from the credit card in 2005 , i must have applied online as theres a box which has been ticked ,it is a computer tick this ticked box is saying i agree to the terms & conditions (there is no signiture just the tick ) There does seem to be lots of info about interest rates & payments etc but my name or any of my info isnt on any of the documents should it be ?
Sorry im being so vague i wish i could post it for someone to look at .
What i really need to know is if i did apply online does this tick in a box mean i have signed for the card ? Also should my name appear on the credit agreement ?

I dont know how to scan it and when ive just looked it does say my name(oh dear i am worried now )0

What i cant understand is they have sent me 2 CCA -they say we enclose a copy of the CCA (original and current )
On the what they call original thats the one with the tick ,this states £25.00 charges if i go over drawn ect .This one has my name on it
The other one is much longer but theres no tick boxes or signiture on this it states there charges will be £12.00 if i go overdrawn .This one doesnt have my name on it .
Why do i have 2 CCA i wonder

The one without your name is a copy of the current terms and conditions, as it refers to a £12 charge.
it is the original terms and conditions that matter.

This is my worry MBNA have not accepted my DMP but they have froze interest infact they have been really understanding (i would say they have accepted my plan because interest is frozen yet i get letters saying you need to increase your payments for us to accept your DMP ).
Last time i spoke to them they said if i did not increase my payments they would sell my debt on which i was quite happy about because i may be in a position to offer F&F settlements soon .
I am panacking now about worse case senorios which would be MBNA taking me to court because i have requested a CCA and i do think that i am one of the unlucky ones because it does look like the CCA is valid .
My debt is for around £2500 do you think they would take me to court for this amount .I am hoping they will do what they have said and sell my debt on, and i hope i have not messed things up for myself with asking for my CCA0 -
I cca,d MBNA on 2 accounts I had with them in feb, had no response at all and they have sold both debts on to 2 different dcas without providing me with anything, I had no notice of assignment from MBNA or no communication from them at all.They had frozen interest and I had never missed a dmp payment but it obviously wasnt enough.0
