Work expenses claim back - what/how/when can i claim?

Ive just heard that if you work, you can claim back expenses such as the following
Mobile Phone
Petrol to and from work
clothing
but i work from home so i am thinking of claiming
Laptop costs
internet
phone
heating
leather chair etc
i heard the proportion i can claim is 1 over the numebr of rooms in my house exlcuding bathrooms?
is there any extras which you lot claim?
how far back can i claim, 4 years back?
could i claim from internet banking print offs (eg mobile phone contract direct debit)?
what form do i use to claim.
