Requestings Statements from Creditors when on a CCCS DMP

Hello everyone! I'm hoping someone wonderful can help me out or point me in the right direction.

I've been on a DMP with the CCCS for a year now, and all is going well, thankfully - the first couple of months were a bit grim but then everything quietened down.

While I continue to get statements from most creditors, some have stopped, usually once the account has defaulted. Egg advised me early on that this wasn't possible but confirmed they were getting my payments.

Last summer, I wrote to Cahoot and RMA Resolve (for Amex) requesting statements. I heard nothing from Cahoot. RMA Resolve called me after I chased my original letter by e-mail - I refused to go through security but did call them back. RMA Resolve confirmed receipt of my payments but said they weren't set up to send out payments.

As an aside, CL Finance (for GE Money) do send out monthly statements.

In most cases, I'm no longer able to check accounts online as the accounts are considered closed.

I did recently get credit reports from Equifax and Experian so I could at least see the outstanding balances. In most cases, they reflected my calculations (so no major worries), but a couple differed so I obviously want to check that out.

The CCCS has advised me that creditors must provide me with a statement or list of all transactions, if requested - but have also advised that some charge a fee for providing this information (anyone know why?). So I'm now going to write to the creditors.

In my letters, I shall refer to my CCCS DMP, my monthly payments, mention any previous correspondence, ask for a statement of all transactions since I last heard from them, and request future statements, preferably monthly. Is there anything I've missed? Anyone know if a template letter exists? (I've no problem with writing a letter but if a great template exists, I could amend that to fit each creditor.)

Many thanks for reading my post - apologies that it's ended up a bit longer than I'd anticipated.
