Just printed my online forms, and noticed a couple fo bits of missed off, but when I have gone back to check they are entered online:-
1. Type of card, I have entered credit card / debit card against each one
2. No Job Title showing the first page of 6.27 but is showing on the first page of 6.28
An suggestions apart from leave as is and complete with pen on the day at court.

Hi dave,
Not so long ago we were all discussing the glitches on the online forms. You have stumbled across one or two of them. No-one seems to be able to get the type of card filled out - I filled it in at court, initialled it and dated it... along with anything else that had either disappeared or worked its way into a different place. The clerk also initialled my changes.

I would suggest that where the information is not shown use section 13 additional information section - the one that does not appear on screen when you are completing your online forms but automatically prints out when you have submitted and printed the forms off. Write the question number in the left section ie 6.27 and put your information/response in the right section of the page and remember to use a pen not pencil and write it in capitals.0

I don't think there is any need to do this with the "type of card" as the boxes are quite clearly there and it is easier to understand if the answers are written in the correct place.
It isn't really additional information, but missing information that nobody seems to be able to print out. The courts are used to it.
IMHO

Does it need to be the type of card, ie visa, mastercard or just credit card sufficient?0

I put visa credit, visa debit, mastercard and so on... but I think credit or debit would do....
