Menu choice template?

Hi,
My Sister is having a choice of three dishes per course and I need to include a menu choice with the invitations that I am making. Does anyone have a template where there are boxes or circles for people to tick their choice and then mail them back with the rsvp. Thanks

Do you have word? if so, type up the menu with the choices and add a circle or a square next to each choice and ask the guest to select what they want to eat.0
