Subject Access Request letter - where is it?

can anyone please point me in the right direction for the letter, i can't seem to find it.

Also, i'm not sure if anyone has any experience of this but I am about to do a SAR for a deceased person who I am acting as executor for. Apart from sending the court document which gives me power of administration is the process just the same?

There may be information on the information commissioner's website

www.ico.gov.uk or www.ico.org.uk (I can't remember which)

Thanks Poor, Down to the £840 then decided to buy a house with the OH!
and just got a letter from BCW stating that I owe them £32.36 for a bill I can prove I have paid - saw them on Watchdog last week.
