On an application fom for the NHS,it asks for two references,one being your current employer.Would they be likely to request this before offering a position ? If so,I could not chance this happening,as I dread to think what would happen at work ! Any advise greatly welcome.

They wouldn't usually contact a referee prior to a job offer, but to make sure write under the name and address "Please do not contact prior to job offer" or something like that.
If the job is subject to CRB then I believe some organisations start the reference process early.0
