Opinions please.

The current situation;
£44500 outstanding with A&L
House value £160000+
Monthly payment Approx £350 (not had all the rate changes yet)
Remaining term 18Years
Annual salary of around £20,000

What we want to do;
Move the mortgage to another lender
Fix for as long as possible at the best rate (10 years maybe)
Reduce the term as much as possible
*possibly* raise another £5000 for some bits n pieces to be done.
Keep the payments under £450.

Problems; credit history not spotless but mortgage payments flawless, never defaulted on a loan or any finance but have been late/missed credit card payments and been over the overdraft limit at the bank. (going to pursue Halifax for overdraft charges *vigorously* in the next few months on hardship grounds)
