Obtaining a Tax Refund from IRS in USA

Has anyone managed to get a tax refund from the IRS in USA?
My son has been trying to do so for over 2 years without success. He made one error on the application form and has been unable to correct it. The IRS in USA keep losing his papers and take months to reply to any correspondence. Also the Embassy people in London are less than helpful.
Any suggestions would be appreciated.
