Requesting holiday pay and P45

hi,
I left a job in November and I am still awaiting my holiday pay and P45. I have written to them once and requested they be forwarded as soon as possible and have never had a reply. Please can someone advise on how to word a letter more forcefully requesting these.
The company is a shambles so i'm not surprised they havent replied really.

P45 is easy, just have a word with the tax people, and they will make sure the company issues a P45 asap, or risk the wrath of the Inland Revenue!
Holiday money, well probably a small claims court, but really can't help here.GOOGLE it before you ask, you'll often save yourself a lot of time.
