Looking for an I And E form

Can anyone point me in the right direction for an I&E form. Helping a relative get their head out of the sand. They have a negitive figure with cccs and the soa on make sense of cards so cccs cannot help. I need and I&E form to send to creditors to demonstrate even token payments are not practical at the moment.
