Hi Steve,
Got my pack in front of me (to be sorted today). I have various information "leaflets":- What will happen to my bank account - two copies for some reason?
- When will my BR end?
- Guide to BR
- The insolvency service charter.
- What happens when you are interviewed by the OR?
- Ethnic monitoring form
- Questionnaire - Comments suggestions about the IS/OR
- Something authorizing the OR access to all my tax details?
- A letter informing me of my duties as a BR to be signed and returned.
There's a load of things on the checklist though over the phone the OR only said she wanted bank/CC statements and wage slips???

Yes, in these forms they asked us to send any credit/bank cards in, cut in half, the last 12 months statements of all debts (which we didn't have, just sent what we had) and all loan agreements and used/unused cheque books.
We had a prepaid address label but no envelope. Scrounged huge envelopes off my dad to fit it all in!:T 434 of 2008 :j0 -
