It seems to say that as mentioned previously you are only entitled to 20 mins per 6 hours worked and that 20 mins should be somewhere around the middle of the 6 hours - so persumably if you work a 12 hour shift you would have two lots of 20 mins breaks around 3 then 9 hours into the shift?

I work in retail and earn minimum wage and we don't get paid for our lunchbreaks.
If you work 71/2 hours a day you are apparently entitled to only one 15 minute break but if you work 8 hours you are entitled to an hour lunchbreak (unpaid) and two 20 minute breaks which are paid!
If you are working 7.5 hours then you are legally entitled to one 20 minute break. Not one 15 minute break0 -

Remember you need to be working longer than 6 hours. As has been stated earlier lots of places have 6 hour shifts in place which means you come into work, do the job for 6 hours and go home. No need for any break of any kind.
In the "statutory instrument " copyright Crown Copyright 1998 states that a person who is required to work more than 6 hours at a stretch is entitled to a rest break of 20 minutes. I have seen this to mean that people can work from 7 until 1230 - have a 1 hour lunch break, thereby exceeding the minimum rest break required, then work from 1330 - 1900. Two 5.5 hour shifts with lunch in between. All perfectly legal.0
